Myofascial syndrome due to temporomandibular joint dysfunction (Costen syndrome): Study of 2500 cases

ABSTRACT
Introduction. TMJ dysfunction is a common problem in both medical and dental practice, and is often overlooked due to the number of
structures that cause pain in the skull and neck.
Objective. To report the clinical observations of myofascial syndrome of origin in ATM
during 25 years.
Patients and methods. 2500 female patients diagnosed with myofascial syndrome of TMJ origin.
Results. Average age
of 26.5 years, average evolution time of 1 year; temporo-cervical pain localization predominantly, pain intensity upper than 7 in VAS scale;
with abnormal closure of incisors, decreased mouth opening, bruxism traces, presence of trigger points in chewing muscles, associated
with emotional problems of anxiety and sleep disturbances; average of 4 doctors from different specialties consulted prior to diagnosis.
Managed with NSAIDs, pregabalin, physiotherapy and manufacture of occlusal guard.
Conclusions. In a young female patient, with
headache and neck pain, take into account the possibility of myofascial syndrome of TMJ origin, if she presents with maximum mouth
opening of less than 38 mm, bruxism traces, trigger points in chewing muscles and emotional problems. Once the diagnosis is confirmed,
the management is conservative, with the orthosis (occlusal guard) being the main treatment factor.
