Usefulness of the cementation with vancomycin in hip arthroplasty with deep infection in one stage surgery

ABSTRACT
Introduction. The total hip arthroplasty, is a surgical procedure that is
useful to relieve pain, to improve movement in patients with coxarthrosis,
and to resolve 31B and 31C (AO/ASIF) fractures. The infection in this patients
it’s a fulminant complication. The treatment in the THR (total hip replacement)
infection goes from local/systemic antibiotic therapy, wound irrigation, deep
irrigation, antibiotic loaded chains of cement, one or two stage exchange with
antibiotic impregnated cement, thru Girdlestone arthroplasty. Methods. 20
patients expedients were analyzed, who was performed primary THR Surgery that
went thru infection, and received a surgical treatment at one stage exchange
with vancomycin impregnated cement, 3 patients couldn’t be able to get the
files, and 1 discharged from de Hospital by himself. A close statistically
relation exist comparing the one stage arthroplasty with the date of the last
outpatient service visit, and the infection eradication. With 0.683 and P >
0.07. Conclusions. The one stage surgery with vancomycin impregnated cement
it’s an effective procedure to eradicate the infection in 14 patients (70%)
of the 20 patients studied with deep infection after primary TH.
