Orthodontic-surgical treatment of maxillary hypoplasia class III malocclusion with combined anterior and bilateral posterior crossbite. Case report

ABSTRACT
One of the challenges of the orthodontist is the need to achieve aesthetic harmony based on dento-skeletal alterations, which are determined by an infinite individual variability in the structure and thickness of soft tissues. The protocol of attention of a patient of 26 years is shown, who reported having a serious problem of self-esteem by his way of eating and smiling. The clinical and radiographic evaluation diagnosed the patient as: hypoplasia of the middle third and skeletal class III, hyperdivergent, bilateral molar relation class III malocclusion, bilateral canine relation class III, bilateral anterior and posterior crossbite, lower middle dental line 1 mm to the right, upper and lower mild crowding, horizontal and vertical bone loss in the mandible arch. Treatment correction of maxillary hypoplasia by orthodontic-surgical treatment, bilateral posterior crossbite, anterior crossbite, upper and lower mild crowding, correct overjet and overbite, obtaining class I molar and bilateral canine, matching skeletal relationships. The treatment was divided into three phases: The preoperative consisted of alignment and leveling along with dentoalveolar expansion using the overlay bow, assisted by cross elastic. For the second (surgical) phase, this was along with an advance Le Fort osteotomy I maxillary of 8 mm, then was a midline reversal to the right. The third postoperative was based on detail and retention. The brackets used were Roth prescription slot 0.022" x 0.028", tubes bondables in 16 and 17 upper and 36 and 47 lower. The retention used was an upper and lower circumferential, time of active treatment three years seven months.REFERENCES
