Clinical outcomes after treatment of congenital scoliosis and multiple skeletal malformations: case report in a dizygotic twin

ABSTRACT
Spina bifida is one of the congenital abnormalities that can affect spine’s morphology. It is due a defective closure of neural tube during embryogenesis where it does not exist a posterior vertebral arches fusion. Otherwise congenital spine’s abnormalities are divided in: defect of vertebral formation or defect of vertebral segmentation. Within the defects of vertebral formation, hemivertebra is the most frequent and one of the main causes of the congenital scoliosis. It remains unknown the origin of these abnormalities, however they are considered as multifactorial with genetic and environmental components. This report purpose is to describe the evolution of a male product of dizygotic twin pregnancy whose mother used proovulatory hormonal treatment before conception. It is remarkable that male’s twin did not present any malformation while he presented several osseous congenital malformations as: spina bifida, hemivertebra, cervicothoracic scoliosis, Sprengel’s deformity and congenital torticollis. He received conservative treatment with orthosis for five years and subsequently a surgical intervention. We here discuss the evolution, development and quality of life during his lifespan. We also highlight the possible relationship between proovulatory treatment used before conception with malformations etiology as well the infrequent treatment given.
