Síndrome febril agudo inespecífico en adultos. El diagnóstico entre influenza y dengue

ABSTRACT
Background: Dengue in Mexico aff ects all its geography.
The 4 viruses in co-circulation provoke different clinical
presentations, even severe cases.
In 2009, a dengue epidemic occurred in the metropolitan
area of Guadalajara, Jalisco, Mexico, followed by the
influenza pandemic due to H1N1pdm09. In the Civil
Hospital of Guadalajara, “Fray Antonio Alcalde”, a
scoring system for dengue was implemented, this
system, along with a system for influenza, helped to
differentiate both pathologies.
Method: Two scoring
systems were applied to adults aged 18 or older who
presented themselves with a nonspecific acute febrile
syndrome (NAFS) during April-December 2009. 5653
patients were evaluated with NAFS, of these 3593 had
a positive score for influenza and 2060 for dengue.
The blood specimens were processed for NS1 antigens
and IgG and IgM serologies for dengue viruses 1-4,
as well as nasopharyngeal secretion samples for RTPCR
for Influenza.
Results: Patients with dengue were
older than those with influenza, 33.75 years vs. 30.73
years (p = 0.027). The duration with fever before going
to medical attention was 3.31 days in patients with
dengue. 2.75 days in patients with influenza (‹0.011).
Fever, headache and myalgias were higher in patients
with dengue than in patients with influenza, 99.4% vs.
85.2%, 98.1% vs. 88.4%, 98.1% vs. 81.9%, respectively.
There was more thrombocytopenia, leukopenia and
lymphopenia ocurring in dengue than in patients with
influenza.
Conclusions: The scoring system in NAFS,
for influenza and dengue is of great clinical utility and
helps in the timely diagnosis and early treatment.
