ABSTRACT
Surgical or endovascular revascularization is the cornerstone of treatment for patients with critical
ischemia, however, there are times when the anatomy and progression of the pathology does not allow
the usual treatment. The aim of this article is to return to the concept of distal arteriovenous fistulas as
management in patients in whom there are no adequate vessels in the distal arterial bed. In this article,
we present a clinical case where this approach of revascularization was performed through distal
endovascular arteriovenous fistula, as well as a review of the current literature on the subject. Distal
arterialization is a viable treatment option in the desert foot, with more and more evidence, which
should be considered in patients where the usual revascularization methods have not been successful.
