Iatrogenic tracheal injuries related to the airway approach

ABSTRACT
Tracheal intubation is a feasible and safe procedure that has endured over decades. It isn’t exempt of risks and complications even on experienced hands. One of this fearful complication is the tracheal injury related to airway management. Tracheal injury can be defined as the loss of continuity of tracheal tissue. According to the anatomic extension can be classified as rupture, tear or complex injury. Risk factors related to this complication have been divided in established, probable and stipulated risk factors. Clinical manifestations may vary among patients, they have variable presentation and may not be present in all cases, that’s why diagnostic auxiliaries are useful such as plain radiographies, computed tomography and respiratory tract endoscopy. The decision between conservative or surgical treatment is controversial and is the fundamental reason because a multidisciplinary approach is mandatory and depends on the severity of the injury and the available resources. Mortality is high and bad outcome factors are the presence of mediastinitis, persistent air leak and recurrent injury.
