Mortality risk factors in descending necrotising acute mediastinitis

ABSTRACT
Introduction: Although acute descending necrotizing mediastinitis is a rare disease, it has a high mortality rate. Usually develops after a cervical or dental infectious process. This study is made with the aim of determining the risk factors associated with mortality of this disease,
Material and methods: Retrospective study of data of patients diagnosed with Acute descending necrotizing mediastinitis in the period between January 2008 until march 2017. We compared the risk factors reported by other authors in our population using bivariate analysis and multiple logistic regression.
Results: The factors significantly associated with mortality were: Septic shock presence and surgical delay (p ‹ 0.05), › 60 years old and opening of the cervicothoracic corridor (p ‹ 0.01) and postoperative care at intensive care unit (p ‹ 0.001). The overall mortality was 13 patients (17.6%).
Conclusions: ADNM is a rare but often fatal infectious process that requires aggressive treatment in units with a high level of specialization and a multidisciplinary team experienced in the medical and surgical management of this type of patient.
