Laparoscopic versus open appendectomy in a private hospital: prospective study

ABSTRACT
Objective: To compare the time, complications and days of
hospital stay according to the technique of appendectomy
performed [laparoscopic (LPC) versus open by exploratory
laparotomy (LAPE) or local incision (LI)].
Material and methods: Prospective study. Patients younger than 16
years old with a diagnosis of acute appendicitis based on
the clinical, laboratories and image studies. Three groups
were formed according to their resolution (LPC, LAPE or IL).
The surgical time (hours), the complication rates (trans- or
postoperative) and the hospitalization days were obtained.
Results: We included 136 patients, 54 with LPC (39.7%), 21
with LAPE (15.4%) and 61 with IL (44.9%). There was one
conversion from LPC to LAPE. There were no differences in
the surgical time among the three techniques (medium: 65,
75 and 60 min, p = 0.28), neither in the rates of complications,
trans-surgical (LPC 5.5%, LAPE 4.8 and IL 1.6%, p = 0.14) or
post-surgical (LPC = 7.5%, LAPE 23.8% and IL = 11.5%, p =
0.14). The median hospitalization days were shorter in LPC (3
versus IL = 3 d and LAPE = 5 d, p = 0.64).
Conclusions: The laparoscopic procedure was as efficient as open surgery in
surgical time, complication rates and days of hospitalization.
