Neuroinfection by Listeria monocytogenes in an immunocompetent patient

ABSTRACT
Meningitis is the inflammation of the meninges and the subarachnoid space that cover the cerebral cortex and parenchyma. Its etiology is commonly infectious and in case of being of bacterial origin, it is associated with a high morbi-mortality, and is considered an infectious emergency that requires immediate medical treatment.
Listeria monocytogenes is the third cause of neuroinfection worldwide. The main risk factor is altered cellular immunity, for example, pregnant patients, neonates, chronic immunosuppression, HIV, over 60 years, cancer, among others. We present the case of a 47 year old previously healthy patient with meningitis due to Listeria monocytogenes. She was admitted to the emergency department for the presentation of headache, dizziness, perioral hypoaesthesia and lateropulsion. Laboratory studies were performed, initiating empirical antimicrobial treatment with ceftriaxone, ampicillin and acyclovir was started at meningeal doses. The cases of meningitis attributed to listeria in an immunocompetent young population, such as the patient presented here, are anecdotal. The diagnostic suspicion and the early start of the treatment are fundamental for a successful outcome.
