Sarcopenia en artritis reumatoide

ABSTRACT
Rheumatoid arthritis (RA) is an autoimmune, systemic disease of unknown etiology that is clinically characterized by a state of chronic inflammation of the synovial joints, pain, ankylosis and deformations leading to different degrees of physical disability. The inflammatory process present in RA, leads to a catabolic state with consumption of nutrients that modify the balance especially of muscle tissue. This catabolism of energy metabolism triggers an increase in metabolic alterations and body composition. About 50% of patients with RA may have a state of rheumatoid cachexia, characterized mainly by decreased skeletal muscle mass, a decrease in strength and physical performance. When these 3 conditions are met, the patient is considered to have sarcopenia. The presence of sarcopenia is not exclusive of RA and can also occur in multiple chronic diseases or it can be a frequent entity in elderly people and this sarcopenia is associated with physical disability, as well as poor quality of life and increased risk of mortality. This review identifies frequency, main mechanisms of production, and characteristics of sarcopenia in RA as well as the current treatment of sarcopenia in these patients.
