Interinstitutional recommendations on regards of diagnosis and treatment of advanced hepatocellular carcinoma

ABSTRACT
Introduction: The hepatocarcinoma (HCC) is a type of tumor that usually presents unfavorable prognosis. In the last decade
important advances has been presented, about the staging, diagnostic and treatment, that have modified the patient´s life
expectancy.
Objetive: To describe and analize the existence evidence about the HCC epidemiology, diagnostic and treatment
in Mexican population.
Material and method: A meeting of medics from different specialties and widely experience in the
management of HCC was convened, whom provided current information on the treatment and diagnosis of this pathology,
in order to generate a guide for the orientation and management of this disease. The exposed recommendations in this review
have been confirmed and evaluated through the GRADE (Grading of Recommendations, Assessment, Development and
Evaluation) system.
Results: To achieve an impact in global and progression-free survival, patients with HCC should receive
multidisciplinary treatment, from the early to the locally advanced stage. Sorafenib use is indicated in patients in locally
advanced stage Child-Pugh A.
Conclusion: The diagnosis and treatment of HCC should be individualized according to what
is shown in this document and should be trans-disciplinaries; the patient must be classified according to the Barcelona
Clinic Liver Cancer scale, as well as determine their score in the Eastern Cooperative Oncology Group and Child-Pugh scales
to achieve an impact on global survival.
