Valor diagnóstico de la citología del lavado y el cepillado bronquial en el cáncer de pulmón

ABSTRACT
Introduction: lung cancer is the most frequent and one of the deadliest in the world. Objective: to evaluate the diagnostic value of bronchial washing and brushing in lung cancer. Method: an observational, descriptive, cross-sectional study was conducted in “Arnaldo Milián Castro” Hospital of the Province of Villa Clara in the period between January 2009 and December 2010. The sample was constituted by 185 patients who were performed bronchoscopy with bronchial biopsy and washing, brushing or both. Results: 76,4% of useful samples were obtained with the bronchial brushing, while with the washing was only 38,8%. The lung cancer variant that was most frequently diagnosed by cytology and biopsy was squamous carcinoma. With bronchial brushing, there were 25,4% of diagnostic errors and 28,8% with washing. The sensitivity was low in bronchial lavage (40%) and 67,5% in brushing, while the specificity was high for both cytological studies (97,2% and 89,5% respectively). The efficacy was 71,2% for washing and 74,6% for bronchial brushing. In the COR curves the washing and brushing showed areas under the curve of 0,69 (poor predictive capacity) and 0,79 (acceptable predictive capacity), respectively. Conclusions: bronchial brushing showed to be a superior method to washing in the diagnosis of lung cancer due to the greater percentage of useful samples, to the mayor coincidence with bronchial biopsy, to a superior overall efficacy and greater predictive capacity.REFERENCES
