Conventional stapedotomy compared to CO2 laser stapedotomy at the Regional Hospital «General Ignacio Zaragoza»

ABSTRACT
Background: Otosclerosis is an osteodystrophy of the human optic capsule and presents with conductive hearing loss, although it may also appear as sensorineural hypoacusis and mixed hearing loss. The treatment of choice is stapedotomy.
Objective: To compare the audiometric results of primary stapedotomy surgery with the conventional technique and the one using CO
2 laser in patients diagnosed with otosclerosis.
Material and methods: Cross-sectional, retrolective, analytical study. Patients with diagnosis of otosclerosis from the Otorhinolaryngology Department of the Regional Hospital «General Ignacio Zaragoza» between 2012 and 2016 were randomly included. The data obtained in the preoperative and postoperative audiometries were analyzed in two by two contingency tables comparing both surgical techniques and a Fisher’s test was performed through GraphPad InStat v. 3.0 for non-parametric data.
Results: A total of 135 stapedotomies were performed, 61 using the conventional technique and 74 using the CO
2 laser technique. The patients were divided into three subgroups according to the characteristics of their hearing loss and the postoperative audiometric results were compared between the two techniques and each group, without finding a statistically significant difference (group 1 p = 0.7090, group 2 p = 0.4098 and group 3 p = 0.3090). Satisfactory results were obtained in 89% (n = 120) of the patients, and there were 15 with residual hearing loss.
Conclusion: In this study, we did not obtain a statistically significant difference in the audiological outcome in patients operated on with any of the surgical techniques.
