Clinic-epidemiological characteristics of new onset epilepsy in adult in Las Tunas municipality

ABSTRACT
Objective: To perform the clinical semiology, electroclinical, etiological and antiepileptic treatment characterization used as first option in adult patients with debut epilepsy.
Methods: A descriptive-longitudinal study was carried out of patients ≥18 years of age, attended in the Neurology Service of the provincial hospital in Las Tunas (Cuba) for one year (1-1-2016 to 31-12-2016), with diagnosis of new onset epilepsy and residency in Las Tunas municipality.
Results: The sample consisted of 72 patients. The incidence rate was 36.2 per 100 000 habitants. Of these 47 (65 %) of the female sex. The age group with the highest incidence was 18-29 years (mean age of 43.3 years, range 19-89 years). The most frequent type of epileptic seizure was generalized (41 patients - 57%). The most common generalized seizure was tonic-clonic (21 patients - 29.1 %). The most frequent focal crisis was the cognitive / emotional with deterioration of consciousness and automatisms. There was one case with epileptic status, which was generalized tonic-clonic. There was epileptiform activity in 48 patients (66.6 %) with focal predominance (14 patients - 19.5 %) and multifocal (7 patients - 9.4%). Eight patients (11.1%) showed generalized paroxysmal activity and 17 patients presented normal EEG (23.6%). The indeterminate group prevailed in the etiological diagnosis (38 cases - 52.7%) followed by vascular (27.7%) and post-traumatic (8.33%).
Conclusions: Epilepsy in female sex, age between 18-29 years, generalized seizures and unknown causes were most frequent. Prospective research with standardized variables of semiological and etiological groups with new onset epilepsy is suggested. This is necessary to develop predictive models of evolution and therapeutic control.
