Hypoalbuminemia and postoperative infection in a serious care unit

ABSTRACT

Objective: To characterize the population according to epidemiological, surgical and hematological variables, as well as to determine the relationship between albumin and the possibility of developing an infection of the surgical site.
Materials and Methods: An observational, descriptive and cross-sectional study was conducted between July 2015 and January 2016, in the Intermediate Care Service of the Saturnino Lora Torres Provincial Hospital in the city of Santiago de Cuba. To the 42 patients studied, samples were taken on admission and progressive surgical follow-up until their discharge.
Results: The average age was 65 years, with a minimum of 52 years and a maximum of 73 years. Male sex prevailed (54.8%), the group belonging to grade II of the American Society of Anesthesiologists (28.5%), emergency interventions (33.4%) of the hepatobiliary type (30.9%), with a surgical time greater than 3h (33.3%). Surgical approach was required in 42.8% of the cases and antimicrobial prophylaxis in 61.9%. Of the 52.4% of patients with hypoalbuminemia, 38.1% developed infection, whose albumin average was 21.5 g / l, with a predictive assessment of infection in 12.6 times.
Conclusions: Surgical site infections predominated in patients under the influence of several risk factors, despite antimicrobial prophylaxis and marked hypoalbuminemia upon admission. The assessment of the predictive capacity of hypoalbuminemia in the presence of infection requires a substantial sensitivity, establishing itself as a possible marker in the intensive care units for these types of patients.
