Endoscopic approach for an intraorbital tumor

ABSTRACT
Introduction: The arteriovenous malformations are alterations of high flow also due
to a vascular dysgenesis.
Clinical case: A 50-year-old female patient is presented with a history of lung cancer,
operated in 2013. In December 2015, she went to the office for left-ear enlargement
and eye pain. Computed tomography showed an intraorbital tumor of 2x2 cm.
Neurophthalmological examination revealed axial exophthalmos and increased
intraocular pressure of the affected eye. Vision and eye movements were normal. With
the objective of removing the tumor and preserving vision, the surgical approach was
decided by the endonasal route with orbital extension, due to the tumor’s proximity to
orbit’s medial wall. This procedure by minimal access is performed for the first time in
the country and has not been reported in the international literature. The tumor was
benign (arteriovenous malformations), according to the pathologist report, so the
metastasis of lung cancer was ruled out. The patient is now stable; no other systemic
alterations were detected. Vision and intraocular pressure are within normal ranges.
Conclusions: the total removal of the orbital tumor by endoscopic approach was
concluded to be efficient and novel.
