Trombosis venosa asociada a trombofilias. Revisión y reporte de casos

ABSTRACT
Introduction. Venous thromboembolic disease (VTE) associated with thrombofilia is a rare entity in
a non-vascular practice.
Objective. To investigate prevalence in our hospital, in our vascular group, review the new literature on
this topic and give recommendations.
Material and methods. We reviewed our cases of VTE associated with thrombophilia (TF) in the last
15 years. Also a prevalence study (descriptive, retrospective, observational) of all requested TF markers
was performed at CMABC during 6 years until 2013. Basic statistic was done with this data. Most recent
literature was reviewed looking for new tendencies related to “whom to screen” and other implications
for the vascular specialist.
Results. 44 cases were found. Age range 6-80 years (average 39); 24 men and 20 women; 10 (22.7%)
had pulmonary embolism (PE); 7 with 2 TF, 17 FV Leiden, 11 antiphospholipid syndrome, 10 with
methylenetetrahydrofolate reductase (MTHFR) mutation, 4 with factor II 20210 mutation, 2 with protein
C deficiency, 1 with protein S deficiency, and 1 with Factor VIII. The sites most affected were: infra-
inguinal in 34, 10 with PE, 7 iliac, 2 cava, 2 axillary 1 mesenteric and 1 renal. Furthermore,
15,485 thrombophilic studies were performed in 3,893 patients and basic statistics was done.
Conclusions. We must be selective in requesting the profile or panel of thrombophilia studies, and suspect
only in cases with recurrent VTE, in young patients with history of abortions and in unusual place
of thrombosis. This report adds data for future studies in this field.
