Central myocardial stunning secondary to ischemic cerebrovascular event. Case report

ABSTRACT
Introduction: Neurogenic myocardial stunning (NMS) is a reversible, sudden myocardial dysfunction secondary to acute brain injury due to an imbalance in the functioning of the autonomic nervous system. Its clinical alterations include acute left ventricular failure, metabolic acidosis, increased cardiac enzymes and type B natriuretic peptide, its treatment is oriented to the underlying neurological process with the objective of maximizing the neurological recovery, which will favor the improvement of the cardiac function.
Objective: We present the case to review current concepts in the pathophysiology and treatment of this entity.
Case report: Woman aged 72 presented to the emergency room with sudden neurological deterioration secondary to cerebrovascular event ischemic, their studies (electrocardiogram, echocardiogram and cardiac biomarkers) documented not symptomatic left ventricular dysfunction, with supportive therapy, has neurological improvement and normalization of cardiac biomarkers.
Conclusions: NMS is a poorly recognized entity, which can worsen the patient’s prognosis if it is not recognized.
