Emphysematous pyelonephritis. Report of two cases underling physiological aspects

ABSTRACT
Emphysematous pyelonephritis (EPN) is an acute and severe infection, that is distinguished by necrosis, abscesses and gas within the kidney’s parenquima, collector system or perinephritic tissue. It’s more frequent in women and diabetic people and hightly lethal when sepsis occurs. Emphysematous pyelonephritis is usually associated with infection by E. coli. We report two autopsy cases of emphysematous pyelonephritis, both presented bubbles over the kidney’s surface, pus and gas within the ureters, and one case had bubbles in the bladder wall. There have been reported only few cases with bilateral kidney involvement and the mechanisms proposed for the development of emphysematous lesions within the urinary system involve risk factors like diabetes mellitus, obstructive uropathy and microorganisms that use different metabolic ways whit gas formation and bubbles that may reach the perinephritic area, collector system, or persist within parenquima. Although knowledgement and treatment about this entity have got better, its patogenia is still bad known. We present a revision of the etiologic factors, clinico-pathologic presentation and the envolved mechanisms proposed to form gas and it’s progress.
