Assesment of guidelines for intracerebral hemorrhage: A systematic review on the methodological quality

ABSTRACT
Introduction: Intracerebral hemorrhage (ICH) is
one of the most devastating types of stroke. In order
to improve the management there are a lot of clinical
practice guidelines (GPC) but no all have the minimum
quality requirements.
Objective: To evaluate the quality of the published
guidelines about diagnosis, treatment and prognosis
of ICH using the AGREE II instrument.
Methods: We conducted a systematic review to
identify CPGs. Based on prespecified inclusion criteria
we choose the eligible ones to be evaluated by the
AGREE II instrument. It was established that a domain
was acceptable if it reached a minimum score of 60%.
Results: A total of ten guidelines of ICH were
identified. Only four were eligible for methodological
quality assessment. The scores for the guideline No.
1, 2, 3 and 4 in the domains’ scope and objectives’,
‘stakeholder involvement’, ‘rigor of development’,
‘clarity of presentation’, ‘relevance’, ‘independence
Editorial ‘were respectively 89%, 72%, 67% and 42%;
61%, 78%, 61% and 22%; 72%, 70%, 40% and 28%;
83%, 78%, 75% and 67%; 50%, 50%, 31% and 29%;
75%, 71%, 25% and 63% and; 75%, 67%, 50% and
50%. The overall score were 75%, 67%, 50% and 50%
respectively.
Conclusion: The overall methodological quality of
the four selected guidelines for ICH was moderate
to high, as assessed by the AGREE II instrument.
CPG No. 1 and 2 showed better performance and we
recommend it to be used in adoption and adaptation
strategies in variable health care systems
