Evaluación de guías para la práctica clínica de hemorragia intracerebral: Una revisión sistemática sobre la calidad metodológica

RESUMEN
Introduccion: La hemorragia intracerebral (HIC) es uno de los tipos
más devastadores de enfermedad cerebrovascular aguda y su manejo
adecuado podría mejorar el pronóstico. Las guías para la práctica
clínica (GPC) con un adecuado desarrollo metodológico sustentan el
manejo adecuado de las diversas patologías prevalentes en cada país.
Objetivo: Conocer cuáles son las GPC para el diagnóstico y
tratamiento de la HIC publicadas actualmente y evaluar su calidad
metodológica mediante el uso del instrumento AGREE II.
Metodos: Se realizó una revisión sistemática para identificar las
GPC de HIC, se seleccionó mediante criterios y se evaluó mediante el
instrumento AGREE II. Se estableció que un dominio era aceptable en
su desarrollo si alcanzaba un puntaje mínimo del 60%.
Resultados: Se identificaron un total de 10 GPC sobre HIC
de los cuales 4 resultaron elegibles para la evaluación de calidad
metodológica. Las GPC evaluadas fueron las de AHA/ASA, ESO,
KSCVS/CRCS y SEN. Los puntajes obtenidos en los dominios ‘alcance
y objetivos’, ‘participación de los implicados’, ‘rigor en la elaboración’,
‘claridad de la presentación’, ‘aplicabilidad’, ‘independencia editorial’
fueron respectivamente para las GPC AHA/ASA, ESO, KSCVS/CRCS
y SEN 89%, 61%, 67% y 42%; 61%, 78%, 61% y 22%; 72%, 70%, 40%
y 28%; 83%, 78%, 75%; 67%; 50%, 50%, 31% y 29%; 75%, 71%, 25%
y 63% y; 75%, 67%, 50% y 50%. En el puntaje global las puntuaciones
fueron de 75%, 67%, 50% y 50% respectivamente.
Conclusiones: La calidad metodológica
general de las GPC para la HIC fue de moderado
a alto según la evaluación realizada usando el
instrumento AGREE II siendo las GPC AHA/ASA y
ESO las que presentaron un mejor desempeño y son
recomendables para su uso mediante estrategias
de adopción y adaptación en los diferentes sistema
de salud.
