Cognitive evoked potentials and tests of attention in the process of diagnosis and clinical follow-up in children with attention deficit / hyperactivity disorder

ABSTRACT
Introduction: In children and adolescent with
diagnosis of attention deficit disorder/hyperactivity
disorder (ADHD) it is essential to identify and
validate neurophysiological quantitative indicators
that provide a better understanding about the
neurobiology and cognitive deficits.
Objetive: To examine the event-related potentials
(ERP) and attention test (AT) measure deficits in
selective attention associated with difficulties in
the development of language functions, reading and
writing, representing potential factors of cognitive
risk in ADHD.
Methods: Retrospective cohort study of 297
medical records of children diagnosed with ADHD
according to DSM-IV criteria in a public hospital. PEC
and TA were analyzed.
Results: ADHD children presented inefficient
results in 80% of visual ERP, 93% of auditory PEC,
66% of visual attention test and 79% of auditory
attention test. Results in both ERPs, between normal
ERP and abnormal ERP, between abnormal ERP by
sex and between types of alterations were statistically
significant. Significant differences wer also observed
between normal AT and abnormal AT, between AT
according to gender and between alteration (error
of omission and commission and response time) on
auditory AT, except in the last two types of alterations
in visual AT. The specific language impairment (SLI)
with AT detects risk probabilities in the diagnosing of
ADHD.
Conclusions: Diagnosis of ADHD is a clinical
challenge with limited time during a typical
consultation in the public health system. For this
reason, when the family and/or school teachers
have diagnostic suspicions, results in ERP and AT
would allow to start therapeutic and/or educational
measures that will reduce their symptoms burden and
incomprehension.
