Teenage pregnancy: obstetric outcomes

ABSTRACT
Introduction: Pregnancy in girls under 19 or adolescents in Mexico is estimated at 21.3% of pregnancies and is associated with increased perinatal morbidity.
Objective: To compare prenatal control and obstetric outcomes among adolescents and adults in a private care institution.
Material and methods: Observational case-control study, which included 54 adolescent pregnat females served between January 2014 and December 2015 and a sample of 108 adults (20 to 34 years old) randomly selected from the same period. Their antecedents of prenatal control (consultations), detection of: urinary tract infection, hypertensive pregnancy disease, gestational diabetes, threat of preterm birth and abortion were analyzed as its resolution: prematurity, abortion, type of birth, obstetric complications, weight of the product and neonatal complications.
Results: Of 1,342 postpartum women, 56 (4%, 95% CI: 3-5%) were adolescents (median = 17 years against adults = 27.9 years). 14.8% (8/54) without prenatal control against 7.4% of the adults (p ‹ 0.001) and received fewer visits (median 7 vs 9, p = 0.001). More hypertensives were detected (7.4% vs 2.9%, p = 0.18) and had more preterm deliveries (11.1% versus 1.9%, p = 0.01). There were no differences in: urinary tract infection, gestational diabetes, intrauterine growth restriction, threats of abortion or preterm birth, abortions, ectopic pregnancies, percentage of cesarean sections and obstetric hemorrhage. Fetal death was similar (2.1% versus 2.8%, p = 1.00). No differences were found in the median neonatal weight between the groups (2,977 g vs 2,981 g, p = 0.99). No newborn had serious complications.
Conclusions: Adolescent mothers showed an increased risk of hypertensive disease and premature resolution of their pregnancy, with less prenatal control.
