Impact of premature progesterone elevation on the application of hCG according to the ovarian response to stimulation cycles IVF-ICSI

ABSTRACT
Background: A premature elevation of serum progesterone level
has been described in some cycles of IVF, despite adequate suppression
of LH surge; this has been related to poor outcome that may vary
depending on the ovarian response to stimulation.
Material and Methods: This is a retrospective controlled cohort
study, which included 663 cycles where embryo transfer was achieved,
from January 2011 to February 2015. Cycles were divided into Study
groups depending on ovarian response to stimulation (Group I: ‹ 5,
5-10 and ›10 oocytes retrieved). For each group implantation, clinical
pregnancy and ongoing pregnancy rates were studied depending
on the level of progesterone using as a cutoff value ‹1.5 ng/mL and
≥ 1.5 ng/mL.
Results: Implantation, clinical pregnancy and ongoing pregnancy were
significantly higher in Group III, regarding P4 level and whith P4 ‹1.5 ng/mL.
Pregnancy rates were higher when P4 was ‹1.5 ng/mL in Groups II and III. No
significant differences were found in the studied rates among Groups with P4
1.5 ng/mL.
Conclusion:The level of progesterone on the day of hCG application
is a prognosis factor for FIV outcome, independent of ovarian response
to stimulation
