Extra-articular manifestations of rheumatoid arthritis

ABSTRACT
Background: Presence of extra-articular manifestations of rheumatoid arthritis occurs in more severe disease.Objetive: To determine the frequency of extra-articular manifestations of 617 patients with rheumatoid arthritis of a university hospital in Mexicali, Baja California, Mexico.
Material and Method: A prospective study in which 617 consecutive rheumatoid arthritis patients were included (549 females) between 2010 and 2015. Two groups were formed: Group A included patients with ≤12 months of symptoms (n=222) and group B, pa- tients with more than 12 months (n=395). Antibodies against cyclic citrullinated peptide (anti-CCP2) were analyzed in early rheumatoid arthritis patients. Rheumatoid factor was measured in both groups. Extra-articular manifestations frequency was investigated by clinical, radiological, serologic and histopathological studies.
Results: The overall frequency of extra-articular manifestations was 26% (157 patients; 19 from group A, with rheumatoid arthritis ≤12 months and 138 from group B, with rheumatoid arthritis ›12 months). The most common extra-articular manifestation was rheumatoid nodules in 16% (17 in group A vs 82 in group B). Rheumatoid factor was positive in 69 women vs 14 men (p=0.17). Anti-CCP2 was present in 12/17 patients with rheumatoid nodules and severe disease (r=0.99, p=0.000). Twenty patients had two or more extra-articular manifestations. Sicca symptoms occurred in 46 patients.
Conclusion: Rheumatoid nodule and sicca symptoms were among the most common extra-articular manifestations associated with autoantibodies and in females. Twenty patients presented two or more extra-articular manifestations.
