Granulomatous mastitis: a challenge for diagnosis

ABSTRACT
Granulomatous mastitis, also called lobular granulomatous mastitis, is a benign disease of low frequence. It is characterized by the presence of chronic inflammation, the gland becomes erythematous, with a palpable mass and is often accompanied by abscesses. It is a disease which is difficult to diagnose since it can be confused with cancer, tuberculosis, sarcoidosis and others; due to this, the definitive diagnosis is histopathological and an excision biopsy is required in all cases. A case of granulomatous mastitis, which occurred in Mexico, is presented, in order to learn about this pathology and facilitate its diagnosis. A 38 year old female patient, without a relevant medical history, began with pain and increased volume of the left breast, which gradually progressed to a generalized induration and abscess formation, erythema and nipple inversion. A CAT scan showed an inflammatory process. The abscess was drain and the purulent discharge was collected, which was cultivated whitout any development. A breast and left axillary lymph node biopsy was taken. Granulomas were reported surrounded by giant stromal cells, lymphocytes, plasma cells, and neutrophils suggesting chronic granulomatous mastitis, negative for malignancy. Treatment with prednisone achieved a complete remission of symptoms. The definitive diagnosis of the granulomatous mastitis is histopathological.
