Ogilvie´s syndrome after cesarean section. Presentation of a clinical case

ABSTRACT
Introduction: ogilvie syndrome is an intense upset that has symptoms, semiology and the radiological aspect of an intense obstruction of the colon without evidence of a mechanical cause. Sir William Heneage Ogilvie described it in 1948.Clinical case: we present a case of Ogilvie´s syndrome after cesarean section in a pregnant woman with 39, 0 weeks, with aggravated pre-eclampsia and dysfunctional childbirth than next to the abdominal progressive postoperative distention and mass radiological dilatation of the colon led us to conservative treatment, maintaining with nasogastric, ano-rectal, partial parenteral nutrition and use of pro-kinetics. The patient's evolution was favorable.
Discussion: the parasympathetic nominates as a candidate a misbalance in the autonomic innervations of the smooth intestinal muscle with predominance of the sympathetic over parasympathetic nerves. Epidural has related with oxytocine perfusion for its influence in the intestinal motility and anesthesia in the obstetric patients. Its diagnosis along with the clinic is radiological. Treatment is conservative in the first 24-48 hours than, if one do not get expected results: more than 12 cm cecal radiological diameter or presence of perforation, a cecostomy is indicated for percutaneously or laparoscopically, which failed to match in our patient.
Conclusion: accomplishing a precocious diagnosis, gives one the possibility of establishing a suitable therapy, in the great majority of the cases it is conservative.
