Neurophysiologic assessment on thoracic outlet syndrome

ABSTRACT
Introduction: the Thoracic Outlet Syndrome (TOS) comprises numerous and
overlaping disorders caused for the compression of brachial plexus and/or vascular
elements to level of the called outlet thoracic.
Objective: to determine electrophysiological alterations, this sensibility and
correlation with means causes of the thoracic outlet syndrome.
Methods: It´s evaluated 30 patients of both sexes with clinical diagnostic of
neurogenic thoracic outlet syndrome, divided in two subgroups (one of nervous
compressive cause demonstrable and other not demonstrable). At all it’s made
electromyography (EMG), motor nervous conduction study (MNCS) and sensitive
nervous conduction study (SNCS) of cubital nervous, SNCS of medial antebraquial
cutaneous nervous (MAC) and somatosensory evoked potentials (SSEP) of medium
and cubital nervous. It’s applied descriptive and inferential statistical technicals to
neurophysiologic variables.
Results: the studies more sensible (positives) were: SNCS of MAC nervous and
EMG (more of the 75 %), follow of the SSEP of the cubital nervous (63, 3 %). The
specifics alterations more frequent were in the amplitude, follow of nervous
conduction velocity and the latency in the nervous conduction studies and in the
SSEP, the Erb potential latency, follow of N13-N20 interval. Not found signifycative
differences of the electrophysiological alterations between both studied groups.
Conclusions: the presence of alterations in the electrophysiology studies (EMG and
SNCS of MAC nervous) of patients with TOS confirm the clinical diagnostic of the
syndrome with a high sensibility, although didn’t was specific to differentiate the
study groups.
