Relación entre hallazgos histopatológicos y complicaciones del muñón en el postoperatorio temprano de amputación supracondílea

ABSTRACT
Material and methods. A retrolective, case and control, descriptive study was performed which involved
52 patients after above-knee amputation due to lower extremity arterial disease. We reviewed slides
from the histopathological study to reclassificate atherosclerotic plaque found at proximal level, determining
if distal arteriolar thickness was present and measuring intima-media thickness at surgical limit
and below-knee vessel in order to establish if those findings and presence of early stump complications
are associated.
Results. We analyzed the records of 52 patients, 26 as control group and 26 as cases, having had surgery
between October 2013 and June 2014. The most frequent stump complication was infection 69%,
dehiscence and necrosis 13% and proximal amputation 4%; regarding most frequent atherosclerotic
plaque was type VII (34%). At the level of the surgical limit the intima-media thickness was 0.8 mm
(30%) and at below-knee vessel level 0.5 mm (21%). After analysis with χ
2 there was no significant statistical
association between atherosclerotic plaque type, intima-media thickness at surgical limit nor
below-knee vessel with complications. Regarding distal arteriolar thickness it was found in 100% of patients
with stump dehiscence and 93% of those infected, P ≤ 0.35.
Conclusions. The histopathological finding of distal arteriolar thickening has a direct impact on the
relative risk of local and early complications in the above knee amputation due to peripheral arterial
disease.
