Anesthetic considerations in patients with amyotrophic lateral sclerosis

ABSTRACT
Introduction: anesthesia was administered to a patient with a neck tumor and antecedents of amyotrophic lateral sclerosis, a neurodegenerative and progressive disease that affects upper and lower motor neuron units and interferes in voluntary activity.Objective: to present the natural history to a patient with amyotrophic lateral sclerosis and the particularities during general anesthesia.
Clinical case: female patient, with 50 kg of weight and a diagnosis of a left neck tumor (exeresis). Antecedents of amyotrophic lateral sclerosis. Patients with this disease who need surgical intervention constitute a challenge for the anesthesiologist, due to the rare frequency of the disease as to be present. The procedure was successfully completed without any relevant anesthetic or surgical complications, with an adequate recovery, while only as postoperative complications abundant accumulation of secretions was present, for difficulty to swallow that was solved by aspiration.
Conclusions: patients with this condition should be paid special attention regarding the use of neuromuscular blocking drugs, then not depolarizing ones are recommended, with the consequent lengthening of the total time. The use of only halogenated is secure or in combination with other anesthetic products. Close attention should be paid to the intraoperative monitoring and particularly to the immediate postoperative time, due to the difficulty of the swallowing process, the secretions and regurgitation frequently produced.
