Effects of upper and lower external oblique muscle desinsertion as an adjuvant to the components separation technique in pigs. Experimental study

ABSTRACT
The components separation technique involves separation of external oblique muscle from rectus abdominis and internal oblique muscles, as well as separation of posterior rectus sheath from the posterior surface of the rectus abdominis muscle. The purpose of this study was to investigate if, in pigs, a greater cephalic and caudal release of the external oblique muscle increases medial mobilization from the rest of the abdominal wall. Ten pigs were studied. After midline laparotomy, three dynamometers were attached to the medial border of the rectus abdominis muscle and constant medial traction of 5 Newton was applied. The medial border of rectus muscle was marked on blue background after each of the components separation maneuvers. Measurements were performed from the midline to each line. Also, the area between the midline and each line was calculated. The differences were analyzed with paired Student’s t-test. A normal distribution of all variables studied was seen. The differences between midline and each line of study were statistically significant (p < 0.0001). The advanced area after study maneuver was 19.03%. When considering each third, the distances of progress after the maneuver of study were 19.52%, 16.72% and 17.04% in the upper, middle, and bottom thirds, respectively. We conclude that upper and lower desinsertion of external oblique muscle significantly increases medial mobilization of the abdominal wall when performing the components separation technique in pigs.REFERENCES
