Aneurysmal bone cyst of cervical spine: surgical management in a 15 year old girl. One year follow up. Case report

ABSTRACT
Aneurysmal bone cyst is a non-neoplastic benign expansive lytic lesion characterized by multiple blood-filled cavities partitioned by connective tissue septa. The location in cervical spine is rare occurring in approximately 2% of all cases, with a predilection to develop in pediatric population, 80% of cases occur un patients under 20 years. We present the treatment for a 15 year old patient, with a history of decreased strength, hypoesthesia in dermatomes C5 to T1 of the right upper limb. Diagnosed by imaging studies as an aneurysmal bone cyst in the body and posterior elements of C5. The lesion was treated with curettage, and 360o instrumentation by a dual approach in one surgical stage with mesh placement, cervical plate, facet screws and posterior bone graft. At a follow-up of one year the patient remains asymptomatic with no signs of recurrence. Accurate diagnosis, early identification of complications and early treatment of this pathology is one of the goals for treatment. Complete resection of the lesion is associated with an excellent prognosis for healing. Due to the high rate of instability, fusion is indicated in most cases.
