Acute pancreatitis as the initial manifestation of systemic lupus erythematosus

ABSTRACT
Pancreatitis as a manifestation of systemic lupus erythematosus (SLE) is rare and rates are less than 5% in published
series in Europe and United States. Diagnosis relies on clinical, serological and imaging findings, and the exclusion of
other common causes of pancreatitis. The mechanism by which a patient with SLE develops pancreatitis is not fully
understood, but it is considered multifactorial, being vasculitis and microthrombosis the main pathogenic mechanisms
involved. Lupus pancreatitis commonly presents in patient with moderate to severa SLE activity and portends a worst
prognosis. Nonetheless, it is highly responsive to steroids, thus, a timely diagnosis is paramount. We present a case of
lupus pancreatitis as the inaugural manifestation of SLE.
