Special considerations for laparoscopic ventral hernia repair of large and multirelapse incisional hernias

ABSTRACT
The repair of giant post- incisional hernias still now in debat concerning the ideal technic. Laparoscopic surgery had show a role on the repair of this type of hernias when is compare with the standard techniques. The present article analyses the consideration to be taken before treatment, specific indication selection of patients and special emphasis on the surgical technique and finally the report of a series of 200 patients that were operated till November 1999, 80 (40%) of them with giant hernias 65 (81%) were recurrent (1-9 occasion) before laparoscopic surgery. None of them require conversion to open surgery. Complication were minor specially seromas that were manage as ambulatory patients. In all the cases PTFE, Gortex Dual Mesh was used, there was not a single case of infection with an average hospital stay of 48 hours. The follow up has been an average of 3 1/2 years with 5 recurrents (2.5%), 3 of them reoperated all of them localized in the left upper quadrant and subxifoid region. The final results show clearly the beneficial advantage of the laparoscopy compared to open standard techniques the basic condition is to have a perfect control of the surgical technique.REFERENCES
