Isthmian-cervical ectopic pregnancy and invasive hydatidiform mole

ABSTRACT
Ectopic pregnancy is defined as the implantation of the blastocyst outside the endometrial cavity and corresponds to 1-2% of all pregnancies. Ectopic pregnancies are implanted in the fallopian tube (in different sites), and rarely in other places like ovary, cervix, abdomen and cesarean scar. We present the case of a 29-year-old patient, gravida 2, with a history of a previous cesarean section; she is admitted to the hospital with a clinical and ultrasonographic diagnosis of incomplete abortion, for conducting instrumented curettage. During the surgical procedure, intense bleeding occurs despite corroborating the absence of placental remains; we perform an laparotomy to rule out unnoticed uterine perforations, without finding evidence of them. We opt to practice total hysterectomy to control bleeding. We view in the area of the uterine isthmus a bulge corresponding to tissue matching macroscopically with chorionic villi, diagnosing ectopic pregnancy localized in the scar tissue of the previous cesarean. When performing the histopathological analysis, the existence of invasive mole is concluded. According to our literature review, this would be the first reported case of the coexistence of invasive mole and ectopic pregnancy in previous cesarean site.
