Difference in binaural masked thresholds (MLD). Basis and controversy for the study of central auditory processes

ABSTRACT
The purpose of this review is to disseminate the main features of the test of difference binaural masked thresholds (MLD for Masking Level Difference), its usefulness, clinical and experimental experiences regarding their anatomo-functional basis, with emphasis on the need to extend the criterion of abnormality in the interpretation of this measurement. The MLD test is little known, in clinic is binaural and useful for the study of the central auditory processes. A background is mentioned based on the mechanisms that are involved. The important peculiar sensibility of the brain to identify the threshold of two stimuli that are anti-phasic or dichotic over other stimuli that are homo-phasic or diotic is emphasized. MLD values that have been used to correlate with changes in the brainstem to find figures under seven decibels are discussed. What the recent medical literature reports on the involvement of cortical levels in the test run, is described. The need to study the performance of this test in clinical models of isolated or associated cortex hearing damage with insular and corpus callosum damage, arises.
