Evaluation of a lateral flow immunochromatographic system for screening of human leptospirosis

ABSTRACT
Introduction: The human leptospirosis is an emergent disease with worldwide
distribution, it present a difficult diagnosis because it has not a specific clinical
symptom. With the purpose of contributing to the early and opportune diagnosis of
this entity in Cuba an immunochromatography system of lateral flow of national
production was evaluated.
Methods: A system indigenous was evaluated, that uses an antigen, formed by a
mixture of complete cells of leptospiras coupled to nitrocellulose membranes and a
conjugated of colloidal gold with anti human IgM, as detection substance. For the
evaluation 215 human serums were studied, divided in two groups, 75 positive
samples (Group l) and 140 negative samples to leptospirosis (Group II). The
Microscopic Agglutination Test (MAT) with living antigens was used as reference
method.
Results: The system demonstrated high values of sensibility (96 %), specificity
(97,1 %) and agreement (96,7 %) according to the system of reference used. The
values positive predictive and negative were of 94,74 and 97,84 % respectively.
The reason of positive verisimilitude was of 33,60 and the negative of 0,04.
Discussion: The immunochromatography system of lateral flow to propose could
constitute a useful tool for the early investigation of the human leptospirosis. It is
recommended its validation and their future introduction in the net of laboratories
of the National System of Health in Cuba.
