Surgical treatment early vs delayed in ankle fractures and the cost implications

ABSTRACT
A study of the time between the diagnosis of an ankle fracture and operative treatment with open reduction internal fixation (ORIF) was undertaken. We assessed
the length subsequent hospital stay, the complication and the cost implications. The delayed in operative treatment was 48 h from injury and was associated with a
lenghtening of stay. Between January-December 2000, 684 ankle fractures requiring ORIF there were type B Weber fracture in 68 % in both groups (early vs delayed).
The patients who had their operation delayed were in hospital for a mean of 9.1 days vs 2.1 days in early group (P < 0.005). There were not differences in complications in both (P > 0.005). The cost in the ankle fracture treatment was less per patient whose operation was early without increasing in complications related. We recommended "fast track" protocols for treatment in these patients.
