Hemipelvectomy in the treatment of the osteosarcoma in adolescents. Demostrative analysis of three possible scenarios

ABSTRACT
Background: Hemipelvectomy and its varieties are uncommon procedures applied to patients carrying musculoskeletal and articular tumors at the level of the pelvic girdle. The specific procedure is determined by the location and extension of the neoplasm to resect. The pelvic girdle is not a frequent location for osteosarcoma.
Objectives: To demonstrate the possibilities of surgical treatment for the osteosarcoma located in the pelvic girdle.
Material and methods: Three cases treated with different kinds of hemipelvectomy in patients with osteosarcomas will be taken. The circumstances of each case will be analyzed, as well as the sequel of each procedure. The classification of resections was made based on the one proposed by Enneking and Dunham in 1978.
Results: Two patients were treated with internal and one with external hemipelvectomy. The internal ones were classified as III and IIA + III, respectively. The external one was classic. The function in the internal hemipelvectomy III is complete, while in the IIA + III is deficient, requiring crutches to wander. The external hemipelvectomy was considered as palliative in an advanced case, with the finality of suppress the pain, in the same way those narcotics; with the external hemipelvectomy it was achieved a better mobilization of the patient.
Discussion: The election for the chosen procedure is individual and has the main goal to control the illness in a local way by leaving free surgical disease margins. The prevalence of the limb is a secondary goal.
Conclusions: The osteosarcoma requires a multidisciplinary approach in specialized centers. Any hemipelvectomy is a mayor procedure that must be realized by an experimented surgical team.
