Influence of the lowering of the nasal pyramid technique (let down) on the middle third face

ABSTRACT
Background: The altered facial volume or deficits in the midface in young patients may be caused by nasal chronic obstructive diseases, which similarly alter the normal proportion of the same, as in the case of tension noses. These cases can benefit by performing a rhinoseptoplasty made by a technique of “nose let down”, which has the positive side effect, the midface filler, specifically the malar region. That is why it is necessary a proper handling of diagnosis of nasal disease and a facial analysis before a surgical planning.
Objective: To describe the influence of the technique of descent of the nasal pyramid (let down) in the midface.
Material and method: A retrospective study was done with a population of 35 patients. We included all patients undergoing primary rhinoplasty on the technique of let down in a period of two years. The measurement was performed on standardized digital photographs, taking the distance between the outer edge and the lower edge aesthetic position orbital determining the most prominent part of the malar eminence.
Results: The average distance of the start of the malar prominence was 12.91 ± 1.01 mm preoperatively and 11.51 ± 1.29 mm postoperatively. The mean difference between the two values was 1.4 ± 0.87 mm.
Conclusion: The influence of the let down on midface allows the mobilization of soft tissues adjacent to the nasal pyramid, and thus filling favorably and naturally this area, giving patients a better facial balance.
