Prevalence of congenital heart diseases associated with Down’s syndrome in a population of Mexico City

ABSTRACT
Introduction: Down’s syndrome is a genetic anomaly with increased morbidity and mortality when associated with a congenital heart disease. The aim of this study was to determine the prevalence of congenital heart disease in Down´s syndrome in Mexico City (altitude: 2,240 meters).
Method: One hundred twenty-seven patients with Down’s syndrome living in Mexico City were evaluated by physical exam, electrocardiogram and echocardiogram.
Results: Congenital heart disease was found in 40%. Patients with congenital heart disease and pulmonary hypertension were classified as having 1) No shunt (n = 18) with systolic pulmonary artery hypertension of 37 ± 9 mmHg; 2) With shunt (n = 33) with systolic pulmonary artery hypertension of 57 ± 29 mmHg (p ≤ 0.001). In those without congenital heart disease or with congenital heart disease without shunt (n = 76), systolic pulmonary artery hypertension was 37 ± 19 mmHg.
Conclusion: Down’s syndrome has a high prevalence of congenital heart disease and pulmonary hypertension. Echocardiography is an indispensible tool for evaluation of Down’s syndrome.
