Behavior of urinary urea nitrogen in the burn patient

ABSTRACT
Rationale: Protein catabolism expresses itself by increased Urinary Urea Nitrogen (UUN) values in burn patients concurring with weight loss. If unatended, protein catabolism might cause fatal complications.
Objective: To describe behavior of UUN in burn patients during admission in a hospital Burn Unit.
Study design: Observational, prospective, analytical.
Material and method: UUN was assayed in 24 hours-urine collections obtained on admission, once hemodynamic reanimation was achieved, and weekly, for 4 weeks; in 28 patients (Males: 53.6%; Age: 46.1 ± 14.9 years; BSA: 37.1 ± 18.3%; Mortality-for-all-causes: 25.0%) assisted at the Burn Service, “Hermanos Ameijeiras” Hospital (Havana City, Cuba), due to burns ≥ 10% of body surface area.
Results: Once hemodynamic stability was achieved, UUN was 12.0 ± 5.8 g.24 hours
-1 on admission (UUN › 5 g.24 hours
-1: 82.1%). After 4 weeks, UUN was 5.8 ± 4.0 g.24 hours
-1 (Δ = -6.2; t = 4.8; p ‹ 0.05). Frequency of UUN › 5 g.24 hours
-1 at closing of the study´s observation window was 46.4% (Δ = -35.7; p ‹ 0.05). UUN on admission was independent from BSA (r
2 = 0.013; p › 0.05). On discharge, average weight loss was -5.5 ± 4.1%. Increased weight loss concurring with UNN diminished values was observed as stay of the patient in the service prolonged (r
2 = 0.763; p › 0.05). UUN behavior was similar in those being alive on discharge as well as those deceased.
Conclusions: Burn patient sustains an important UNN excretion from the very moment of admission to a Burn Unit. UUN is independent from BSA. UUN excreted values diminish progressively as patient´s stay in the service prolongs, while body weight loss is increased. Patient´s condition on discharge is independent from UNN excretion.
