Questionnaire for differential diagnosis between asthma and chronic obstructive pulmonary disease

ABSTRACT
Objective: Determine the reproducibility and utility of a questionnaire designed to distinguish between asthma and COPD. Material and methods: The questionnaire, consisting of 7 questiones, was applied to 12 asthmatic and 12 COPD patients. Maximum grade was 33 and minimum 3. Asthma was diagnosed in patients with no history of smoking habit (SM) nor exposure to wood smoke (EWS) as FEV1 ≥ 70%p, FEV1/FVC ≥ 70% and reversibility to bronchodilation ≥ 15%, COPD was diagnosed when there was a history of SM and/or EWS with FEV1/FVC ‹ 70%, reversibility to bronchodilation › 15%, and no diagnosis of other pulmonary diseases. The correlation coefficient (CC) and coefficient of variation (CV) were determined. ROC curves were constructed to determine sensitivity (S) and specificity (E).
Results: Intraobserver CV was 3% in asthmatics and 8.7% in COPD patients, while interobserver CV was 6 and 7.4% respectively. The R1 intraobserver coefficient in asthmatics was grater than 0.80, the interobserver was 0.48, and for COPD they were 0.90 and 0.85, respectively. Analysis of the ROC curve showed that the best cut-off point to distinguish asthmatics from COPD patients was 18, with S of 100% and E of 91%.
Conclusions: Results of this study show that the questionnaire is sensitive and specific to differentiate asthmatic from COPD patients.
