Usefulness of methylmethacrylate in the giant cell tumor of bone

ABSTRACT
Osseous giant-cell tumor is a neoplasia that presents a malignant potential, a high recurrence index, and probabilities of pulmonary metastasis, specially when it is treated in an inadequate way and/or through radiotherapy. We performed a retrospective, observational, longitudinal, and descriptive study on 35 patients ranging from 15 to 75 years old, mostly from the female sex, with a 1.3:1 ratio. The principally affected area was the femur (thigh-bone) distal third and the tibia (shinbone) proximal third in 80% of the cases. It was practiced intracapsular resection with a methylmethacrylate support in 31 cases and marginal resection with drew-sustained methylmethacrylate in the four remaining cases. Local relapse of the lesion was observed in nine patients (25.7%) between 6 and 16 months of the postoperative period. These cases were treated through knee arthrodesis or replacement arthroplasty with non-conventional prosthesis. One of the patients suffered from pulmonary metastasis and, because of it, he died 36 months after the treatment of the local relapse. The results, based upon the International Society of the Musculoskeletal System criteria, were excellent in 27 (77.1%) of the cases, and good in eight of them (22.9%). The methylmethacrylate support after the intracapsular or marginal lesion reception constitutes an efficient, economical, versatile, and secure alternative that allows the use of other therapeutic options in case of relapse.
