Auditory neuropathy spectrum disorder (ANSD). Case report

ABSTRACT

Auditory neuropathy spectrum disorder (ANSD) is characterized by absent or abnormal auditory brain stem response, otoacoustic emissions and/or cochlear microphonic, absent stapedial reflexes and disproportionate phonemic discrimination in pure tone audiometry. This disorder is more common than it was previously thought, hence the importance of identifying this pathology, which may go unnoticed even by specialists. We report the case of a 2 year, 6 month-old girl with suspected hearing loss, detected by parents at 2 years of age, of stable evolution, identified by inconsistent responses to high intensity sounds, delayed language development (monosyllables like “ma” and “pa”), and communication using shouts, signs, gestures and pointing to the desired object, also showing frustration when not understood. We performed behavioral observation audiometry in the free field, without observing response to maximum intensities, brain-stem auditory-evoked potentials without response to maximum intensities, present otoacoustic emission, and absent stapedial reflexes bilaterally. Hearing aids were indicated, with rejection in the right ear and acceptance in the left ear. After a year, we documented a decrease in otoacoustic emissions reproducibility.
