Swallowing disorder as finding in adult patients with diabetes mellitus type 2

ABSTRACT
Background: Until now, dysphagia in diabetes mellitus 2 is described as
a silent process, but its early detection by screening methods is important
to assess better the existence of diabetic polyneuropathy.
Objective: To determine the association between swallowing disorders
in oral and pharyngeal phase and diabetes mellitus 2 in asymptomatic
adults assisted at Hospital Español de México.
Material and method: A cohort, prospective, controlled study was done
including patients older than 18 years, assisted at Otorhinolaryngology
Service of Hospital Español de México from October to December
2013, with previous diagnosis of diabetes mellitus 2. Patients without
diabetes mellitus type 2 integrated control group. A dynamic test of
swallowing with nasofibrolaryngoscopy was performed to both groups,
cases of aspiration-not aspiration and sensitivity of epiglottis were
detected. Time of evolution of diabetes mellitus type 2 was assessed
in each patient as well as the control of glycemia by quantification
of serum HbA1c.
Results: 25 patients with diabetes mellitus type 2 and 25 subjects
without diabetes mellitus type 2 were included. We found data of
food aspiration towards airway as swallowing disorder in elderly
patients with diabetes mellitus 2 (n=3, p=0.0424). In patients with
diabetes mellitus and glycemic descontrol (based on HbA1c › 6.6%)
we found an important number of patients with laryngeal sensitivity
disorder (n=9, p=0.0492).
Conclusions: There is association between diabetes and laryngeal sensitivity
disorders. As regard its relation with swallowing disorders, cases
communicated were also old, thus, there is doubt if these swallowing
disorders are more related to age than to diabetes mellitus.
