Quantitative Immunohistochemical Study of Proliferation Index and Apoptosis Index in Acné Vulgaris

ABSTRACT
Acné vulgaris is a multifactorial common disease involving the pilosebaceous follicle. Hyperkeratinization in the infrainfundibulum constitutes the
primary element in he formation of comedones. We studied skin biopsies of 20 patients with acné vulgaris with closed comedo or microcystic
(Whitehead) for immunohistohemistry for determination of the Proliferation Index with PCNA and the apoptosis Index with TUNEL tecniques. We
compared with 5 biopsies with normal skin without acné for control. Only we studied this types comedones, not the open comedones (Blackhead)
because they have different clinical and histophatological features. We studied three different areas of the epithelium of the pilosebaceous
unit: epidermis of the surface close to the ostium, in the infrainfundibulum and the sebaceuos duct. The results showed that both índices were
significantly increased evaluated at the epithelium infundibular and the excreting duct of the sebaceous gland compared to the epidermis close to
the ostium. We suggest that in cystic or close comedo exhibit a greater production and mitotic activity and a greater rate of cell apoptosis leading
to an increased in desquamation “ hyperkeratosis” in all the pilosebaceous follicle than in acné free. This means that the infrainfundibulum, the
duct sebaceous and the gland operate and responde as a similar unit to form this types of comedo. It is very important the participation of the duct
to produce the hyperkeratinization in the firsts stages of this close comedo as in the infrainfundibulum. For the contrary, this event not succeed
in the first etages in the open comedo.
