Evaluation of a clinical practice guide for the obstetric patient gravely sick for sepsis

ABSTRACT
Introduction: The sepsis appears among the first causes of maternal morbimortality
and its incidence increased in the last years in the country. A clinical practice guide for
the obstetric patient gravely sick for sepsis doesn't exist in our means. The guides based
on evidences and consent of opinions permit a better therapeutic handling guaranteeing
a progressive and uniform attention.
Objective: Valuing the utility of the clinical practice
guide as a therapeutic rule for the obstetric patient gravely sick for sepsis. Ambit.
Intensive care unit of the hospital Commander Faustino Pérez in Matanzas.
Method:
Longitudinal, prospective and observational study. It was obtained a consecutive sample
of all the obstetric patients gravely sick for sepsis (not influenza) that were admitted in
the period 2006-2012 in the intensive care unit of the hospital Faustino Pérez where the
Guide as a therapeutic rule was applied. The obtained results were compared with
similar data of the intensive care unit of the hospital José R. López Tabrane of the
period 1993-2002 where the therapy was the criterion of the doctors of attendance.
Results: In the period 2006-2012 decreased the mortality for serious sepsis to the 17%;
the evolution diminished to severe sepsis (30,7%) with minor mortality (1,9%); and
although there were more cases (13 patients) the death for septic clash descended to
the 23%.
Conclusions: The use of the guide in the obstetric patient gravely sick for
sepsis is an appropriate therapeutic rule that reduces the mortality.
