Valoration of a scale of multiorganic disfunction as a presage of mortality in an intensive care

ABSTRACT
Introduction: The Sequential Organ Failure Assessment describes the degree of
disfunction of organs in time, and evaluates the morbility of the septic patients, it could
be applied in not septic patient.
Objective: Evaluating the utility as a pronostic scale of mortality of the Sequential Organ
Failure Assessment (SOFA).
Method: Longitudinal and prospective study with 303 patients of the UCI8b of the
Hospital Hermanos Ameijeiras (February/ 2010-agosto/ 2011). The scales APACHE II,
SAPS-3 and SOFA were applied to the admission. The association between SOFA with
the state to the egress was valued with the test [t-Student], with a level of p‹ 0,05. It was
evaluated the predictor power of mortality by means of the area under the curve ROC of
the scales. For the correlation between these, the index of Pearson was used,
considering that strong correlation between them exists if the value of the probability is
minor of 0,01.
Results: The average age: 59.7 years, average stay: 9.8 days; mortality: 39,9% it was
ventilated the 38.3% of the patients and the 64% presented any stay of the sepsis. It
was found difference between the half of the SOFA for alive and dead (p= 0.000). The
area under the curves ROC was: SAPS-3 (0,766); APACHE-II (0,772); SOFA (0,782).
The points of court for SOFA: 2,5-4,5 points. The combination of SOFA/ APACHE II,
showed the greatest coefficient of correlation.
Conclusions: It was proved the utility of the SOFA as a presage of mortality. The used
scales presented an equivalent predictive value. The association of II-SOFA APACHE
obtained a greater predictive value.
